HA 230I RBS Polymorphism Potential for Europe

Current trending indicates a reportable potential for the M230I polymorphism spreading on the Hemagglutinin of PF11. This Receptor Binding Domain change may enhance Vaccine Escape from the currently selected vaccine target candidate, CA/07 X181. 

For those who are following the prediction on February 25, 2010, the pandemic reservoir now shows multiple instances of multiple encodings for 230I.  Model adjustment and data transparency allowed a second set of detailed geographic predictions on 2010-08-09 that have also found traction.

The 230I bearing sequences meeting the prediction are documented in the detailed discussion on Vaccine Escape that demonstrates a 100% change rate in the pandemic influenza (pH1N1) reservoir at the critical HA genetics range between amino acid positions 186 and 248.  89% of the amino acid positions have notated revisions.

Expectations for the M230I polymorphism, that first came to our notice for zoonotic concern on the H5N1 human fatality cluster, have now been revised based on the most current public data.  The GeneWurx model approximates that HA 230I will appear in the PF11 RBS according to the following geographic probabilities.

  • 75% probability in Germany sampled by 2010-12-01.
  • 75% probability in Spain sampled by 2010-12-01.
  • 85% probability sampled by 2010-12-01 in one of:
    • Denmark
    • Norway
    • Sweden
  • 5% or less probability of HA 230I within 60 days of conserving across PF11.
  • 7% probability of HA 230I within 180 days of conserving on one or more Hydrae.
This HA polymorphism guided an investigation suggesting that  anti-viral resistance from the NA H275Y revision is not the only genetic concern with over-usage.  Anti-viral drug usage may drive 230I genetic acquisition, with a higher potential among children and young adults (0-24 years old). The sparsity of complete sequences, discounted by the lack of required meta-data, allows only a rough postulation on the mechanism.

An accelerated viral self-revision, subsequently moving toward M230I, may be modulated by the substantially higher rates of host exposure in the lower age brackets. Additionally, that group’s multiplicity of exposure to a wider variation of influenza strains may factor into the host response/viral revision equation. In plain terms, the viral reservoir is being forced toward hard labour in order to infect and re-infect the youth.

Just as hard work builds muscle, viral exertion against a complex host response challenges the reservoir subset to revise toward an optimal solution. 230I is an excellent and proven tertiary response for a virus on this type of offensive.

These probabilities will be updated as additional data is made public.  Transparency at this post-pandemic stage is essential to formulate viable responses for the risk groups.  Release of sequences and clinical data of a finer detail and higher quantity will allow information-based decisions.

HA 230I Documented in Texas with 225E RBD Revision

The United States Air Force School of Aerospace Medicine published a sequence on 2010-08-20 with multiple RBD changes sampled on 2009-10-04 from an 18 year old male in Texas.  This TexasAF2579_2009_10_04 sequence meets the geographic criteria and genetic change prediction published on 2010-08-09.  The HA 230I in this case is combined with a second RBD polymorphism, 225E.

Pune India Communicates Pandemic Genetic CrossTalk with England and United States

The practice of science exists to serve mankind by discovering answers.

India is in the grip of a post-pandemic acceleration of death due to burgeoning caseloads and an apparent change in viral behaviour.  They have experience, foresight, materials and initiative, but the disease is overcoming the system and killing the citizens.  The health purveyors are doing everything by the book, but the chapters are out-dated apparently.

4,069 People Began TamiFlu Treatment on Monday in Pune India

If someone had told the citizens that the virus would be post-pandemic on Thursday, they may have asked for a treatment deferral . . . unless they trusted their own eyes.  No press release can revise the rise of a IDEARRV disease like the ΣPF11 influenza reservoir.

The facts speak for themselves.

On Monday, 2010-08-09, more than 4,000 citizens were placed on TamiFlu treatment in the city of Pune, India.  83 deaths were recorded from pre-post-pandemic influenza for the prior 7 days in India. 

For the country of India, the weekly average confirmed Influenza death count over the past 14 months is slightly above 30 deaths per week.  The fatality count from the past week of 83 is 2.7 times the average rate, a substantial acceleration.  The Case Fatality Rate varies from 8.8% to 12.1% in the city/district based on the source of data and date bounding. Roughly 1 in 10 people with a confirmed influenza test die in India.

More than 75% of the deaths are in the 20-49 year old age bracket according to the raw data we have been able to reference, with reports from others as low as 70%Years of Life Lost calculations defy all "normal" influenza trends.  Traditional influenza has a CFR of less than half a percent and more than 98% of that half percent are over 65 or under 2 years old. These numbers from India in 2010 are not suggestive of a post-pandemic status.

Perhaps we should all just re-base now that we have read the press releases and settle ourselves into this post-pandemic era.

Step 1. We could  call 83 deaths per week the new "normal" and post that idea as the rate for "seasonal" influenza.  Our forms and percentages will then be quite less concerning to the public.  Then we could take a note from the public health textbooks and "estimate using the most advanced models" a normal CFR and game the model variables until we evaluate to the current 8.8% CFR of India. 

Step 2. Combine the power of the press with the power of the "expert" credential, even perhaps a committee of experts?  In a few pen strokes and an hour here and there behind a microphone, the public would be assuaged and all could return to "business as usual".

Or we could hear the public whose health is suffering, read the data and then tell the truth.

One of these things is not like the other?

Though we'd like to see the sequences from these fatal cases that are currently accelerating throughout India, a review of the data on file will allow insight into the building / budding process of a pandemic virus into and out of a monsoon climate. An array of FlightPaths may be detected that led to Hydra strains downstream in time. We are under no uncertainty that the viral reservoir will follow a similar route this year.

India, last year, was a testing field for PF11. In a nutshell, the sub-continent had a depth of uptake from Asia and North America that transformed and promoted to Europe and then returned to the United States. That pathway is a generalisation / characterisation and is not comprehensive.  Suspicions do lead to a repeat of last year.

Has, perhaps, the 225G on the HA and the 275Y on the NA become more substantial in India after travelling through the world? Is HA 225N playing a role in the more rapid expirations, while cross-linked 225G causes the lingering 3-5 day, ICU-bound feats of ultimate exhaustion? Is a deep, immune escape Hydra carrying HA 165N and 230I at work in the districts showing a 20-29% Case Fatality Rate?

We watch as vaccinated doctors are admitted to critical care wards, as hospitals with advanced experience and materials lose patients daily to Influenza, even upon using immediate TamiFlu blanketing.  Reports of fatalities associated with caregivers, including doctors, are demonstrated in multiple geographies.

Ukraine 502K Marker from Cross-Linked Fatality Documented Worldwide, including England, the United States, Australia and Singapore

The UK Centre for Infections, Health Protection Agency provided a very large deposit marked 2010-08-12 this week. Among numerous RnR * examples describing flow of particular genetic markers, one change from the Ukraine high fatality circumstance of late 2009 becomes documented in England.

The HA 502K found on the UkrTernopilN10_2009_10_28_xL_f cross-linked fatality insinuated into England in only days after this sentinel Ukraine infection was sampled and isolated. A downstream HA revision, 504N, is also documented in this same deposit as appearing widely in England with one instance in Scotland.

Apparently, friends travel together.

225A Appears In Madrid Spain Pandemic Influenza

The very effective contributors to The NIAID Influenza Genome Sequencing Consortium deposited a contemporaneous set of sequences at GISAID this week covering a significant number of data points over geography and emerging trends in pandemic RnR * polymorphism.

225A is found for the first time on an unmixed trace in this pandemic on Madrid296_2009_11_17, having previously been discussed on an unusual sequence from the Ukraine. The HA of UkrKyiv377_2009_11_07_xL only revised at two protein positions, the syn413K for cross-linkage and amino residue 225 (rvT coding up to 6 values).  These multiple, mixed traces at the first and second nucleotides of the codon represent the only documented prior instance of potential 225A in this pandemic reservoir.

Additionally, a narrow slice of the data in this directed review further suggests a peculiar behaviour that our team has postulated in the past 3 months within this present pandemic reservoir (PF11). Spain and Greece provide an active working ground for this particular genetic acquisition cycle behaviour.

Notice that the reservoir appears to individually conserve polymorphisms on strains having few revisions (generalised / consensus strains) from time to time, even after the given polymorphism has appeared on a significant count of sequences having numerous polymorphisms.  This apparent reversal of progressive genetic acquisition may have purpose. 

Preservation behaviour for future recombination using / by creating a type of "universal donor" template is suggested.  This step-wise method of storing an important piece of data for later usage in a labile format is not entirely novel in biology.  Ease of access / transfer / adoption may be increased as a function of the homology / identity of the two interchanging viral strains.  “Mix and match” becomes more organised and discrete in this scenario. Ergo, a generalised virus with only a single revision or a minimal revision count may be more successful as an overlay for material transfer.

Note that Madrid300_2009_11_30 carries only the 373H.  Similarly, notice that syn372Q (in a similar domain) is found on Russian fatality sequences that have no change other than the cross-linkage syn413K.  The syn372Q is also found upon a 225G and a fatal 225N case in the same area, Ulyanovsk

Is a similar function within the same sequence area (372, 373) being preserved in Spain, Greece, Portugal, Belgium, Germany and Australia using the 373H?

Is the Madrid296_2009_11_17 pairing of 225A (in its first recorded occurrence) with 373H another of the reservoir's conservation storage templates that will be referenced for later antigenic escape?
